Changelog

A running record of the latest improvements to WhatsTheDx.

Your clinic opens to everyone

Platform

WhatsTheDx is consolidating around three threads: play daily cases, earn rewards, and build your clinic — with leaderboards and Dx Party as the social layer.

What's new

  • The 3D clinic is now free for every signed-in player at /clinic. Customize your space, and watch the floor busy up as you solve cases.
  • The Dx Office management sim, Clinical Brain review queue, and Million Patients page were retired to keep the platform focused; every turtle shell you have found stays in your collection permanently.
  • WhatsTheDx Pro is now called Dx+ — the same plan at the same price, with a shorter name. Existing subscriptions carry over automatically.
  • Dx Party is now in the menu for everyone, and Dx+ keeps its specialty decks, unlimited Dx Simulator with reasoning reviews, Workup V2 testing, unpublished cases, and specialty insights.
  • Dx Drops are live: solve cases to fill your shipment meter, open drops in your collection, and place what you find — clinic equipment, clinical artifacts, even classic turtle shells — in your clinic.

Case revisions from player feedback

Content

Every case players flagged as clinically unclear was re-read, and the ones that needed fixing were revised.

What's new

  • The abdominal aortic aneurysm case no longer gives away the pulsatile mass in its opening clue — that finding now appears where it belongs, on examination.
  • The body dysmorphic disorder case now spells out what rules out a delusional disorder, and offers that diagnosis as a fourth option in Easy mode.
  • The pharmacy and psychology alcohol cases no longer share identical laboratory values, so the two stop reading as the same case.
  • Accepted answers were corrected on two cases: "ear barotrauma" and "middle ear barotrauma" now count on the barotrauma case, and ischial bursitis is no longer accepted for hamstring syndrome.
  • Two Spanish translation errors were fixed, including one that referred to quitting smoking in an alcohol case.

Solve streaks and turtle shell collectibles

Gameplay

Keep solving cases in a row to build your streak and discover a randomized turtle shell for your collection.

What's new

  • Every scored solve while signed in extends your global solve streak and awards one server-generated shell; an unsuccessful scored case resets the current streak.
  • Eight shell families span Common, Uncommon, Rare, and Mythic tiers; every collectible uses its own visual seed to vary colors, markings, motif layout, and texture.
  • A new CanvasUI-powered reveal celebrates each discovery while respecting reduced-motion preferences, and practice replays cannot be used to farm rewards.
  • The new collection page lets you choose a favorite shell to display beside your streak on specialty and community leaderboards.

Rare confetti celebrations

Gameplay

Correct diagnoses now come with a refreshed confetti burst and a chance to unlock an extra-special golden celebration.

What's new

  • Every eligible first-time solve has a 5% chance to trigger a rare shower of golden stars, complete with a warm glow and a special result badge.
  • A rare celebration is guaranteed by your 20th eligible solve without one, and your progress toward it stays saved on your device.
  • Optional solve sounds can be enabled from the accessibility menu, with a short success chime and a distinct shimmer for rare confetti.
  • Practice replays cannot be used to farm rare celebrations, and reduced-motion preferences continue to skip falling confetti.

Diagnostic Simulator beta

Beta

A new case experience lets you interview the patient, build a ranked differential, and commit your diagnostic reasoning.

What's new

  • Signed-in players can start one free simulator case per UTC day, while Pro members get unlimited cases.
  • Ask focused questions about the history, symptoms, medications, examination, and tests instead of revealing clues through incorrect guesses.
  • Build and rank a differential of two to six diagnoses, then commit it to see whether the final diagnosis was included and where it ranked.
  • A completion feedback widget now captures structured ratings and optional privacy-conscious comments to improve the beta.
  • Every published case works through a deterministic clue adapter, with 12 bilingual authored interviews now spanning all nine active specialties.
  • The simulator is a separate, non-scoring experience, so it does not change the original case result, streak, points, or leaderboard position.

Faster pages, visual refinements, and more Pro cases

Performance

WhatsTheDx now starts faster and feels smoother while keeping its background videos, with more unpublished cases ready for Pro early access.

What's new

  • Background videos now load more efficiently and transition cleanly from the welcome screen into the main experience, without removing the motion and atmosphere of the site.
  • Visual adjustments improve loading states, spacing, responsive behavior, and polish across the landing experience and navigation.
  • Pages do less work up front by delaying nonessential route downloads and below-the-fold rendering until they are needed.
  • New unpublished cases have been added to the Pro backlog, giving Pro members more cases to practice before their public daily release.

Optometry cases are live

Specialties

Optometry is now a full daily track on WhatsTheDx, with its own cases, archive, and leaderboard — and a direct link to the OptoConnections community.

What's new

  • A new optometry case every day, covering glaucoma, cornea, retina, uveitis, and neuro-ophthalmic presentations, in English and Spanish.
  • Optometry now has its own archive, leaderboard, and streak tracking, and can be pinned as your default specialty.
  • Pro covers optometry on the same plan as every other specialty — no add-ons and no per-specialty upgrades.
  • OptoConnections is linked right from the optometry page, so you can jump to the wider optometry community any time.

Accessibility controls and Spanish support

Accessibility

WhatsTheDx is now easier to personalize and available in Spanish across the core experience.

What's new

  • A new accessibility menu brings larger text, higher contrast, reduced motion, a more readable font, and light, dark, or system themes together in one place.
  • Preferences are available from desktop and mobile navigation, use accessible controls, and stay saved on your device.
  • Switch between English and Spanish at any time. Your choice persists, while first-time visits can follow your browser language.
  • Navigation, gameplay, archives, leaderboards, accounts, Pro, Dx Party, and policy pages are translated, with Spanish clinical clues, diagnoses, and explanations shown when available.

Dx Party beta is here

Dx Party

Bring your study crew together for a private multiplayer card game built around wildly fictional clinical prompts.

What's new

  • Host a table, choose a deck, and share the six-character room code with up to seven friends. Guests can join with only a display name.
  • Each round, the reader reveals a prompt, players submit diagnosis cards anonymously, and the reader awards the point before the role rotates.
  • Signed-in players can host unlimited General Admission tables. Pro unlocks specialty decks.

Specialty leaderboards are live

Leaderboards

Every specialty now has its own leaderboard, so you can see exactly how your play stacks up against everyone else in your field.

What's new

  • Boards are ranked by points — solve the daily case in fewer guesses to score higher, with Hard solves worth up to 60 and Easy solves 5.
  • Switch any board between Today, This week, and All-time to see the current leaders or the all-time greats.
  • Set a public display name to appear by name, or stay Anonymous — either way your rank still counts, and Pro members get a badge.
  • You’ll always see your own standing, even when you land outside the top 10.

July 9, 2026

Pin your default specialty

Accounts

Signed-in users can choose a home specialty so the app opens right to the field you care about.

What's new

  • Set any specialty as your default from the game, archive, or leaderboards.
  • Your dashboard, archive, and leaderboards lead with your default instead of a generic pick.
  • Change or clear it anytime — it’s a one-tap toggle.

July 8, 2026

More control over your Pro membership

Pro

Pro members get early access to unpublished cases, a way to practice past cases, and a simpler way to manage billing.

What's new

  • Play unpublished cases before their public release so you can practice ahead of the daily schedule.
  • Replay any case you’ve already finished for extra practice — your points, streak, and history stay exactly as they were the first time.
  • Manage your subscription in a couple of taps: update your payment details or cancel anytime from your account or the Pro page.
  • Secure checkout and billing are handled by Stripe end to end.

July 8, 2026

Archive, dashboard, and difficulty choices

Gameplay

You now have more ways to practice, pick your challenge level, and jump back into cases when signed in.

What's new

  • The new case archive lets you browse past cases by specialty, revisit the latest case, and unlock the full archive with Pro.
  • Signed-in users now land on a personalized dashboard with quick access to their recent play and specialties.
  • Before your first guess, choose Hard mode for free-text diagnosis or switch to Easy mode for all clues and a single multiple-choice answer.

July 7, 2026

A smoother sign-in experience

Accounts

Signing in and managing your account is now faster and more reliable across the whole site.

What's new

  • Stay signed in as you move between the game, your play history, checkout, and your account — no surprise logouts.
  • Sign in or create an account from dedicated pages, or use the quick pop-up right from the menu.
  • When you’re signed in, the menu puts your account and play history one tap away; when you’re signed out, sign-in and sign-up buttons are always clear.

Clearer policies and subscription details

We refreshed our policy pages and Pro subscription details so they’re easier to read and understand.

What's new

  • Our Privacy and Terms pages are easier to read, with quick section jumping on your phone.
  • Subscription details now spell out monthly access, lifetime access, how to cancel, and secure payment.
  • Handy footer links keep our policies a tap away without getting in the way of the game.
